Program/Service Notes:
The Family Foundations component offers pregnant women and new parents appropriate, supportive services through intensive home visitation services. A universal risk screening on pregnant women, and/or families with newborns is completed during intake. Trained staff will work with families to achieve the following outcomes: * To promote optimal child health/development and school readiness * To improve family coping skills and functioning * To promote positive parenting skills and family interaction * To prevent child abuse/neglect, infant mortality and morbidity The Family Foundations model uses paraprofessional family service workers (home care aides) in conjunction with professionals (social workers and registered nurses) to provide home visits to families. In this project, staff have completed HOPES training, according to the Healthy Families America Curriculum.
