Shelter + Care is a grant designed to combine rental assistance with matching supportive social services for a period of five (5) years. It is funded through the Fort Dodge Housing Agency as a grant program from the U.S. Department of Housing and Urban Development.
Shelter + Care's target population includes: 1) Individuals (age 18 or older) that are homeless; and 2) are mentally ill, physically disabled, have chronic problems with alcohol, drugs or both, or have AIDS or other related diseases.
Shelter+ Care rental assistance is sponsor based. Housing units are leased by the Community Health Center. They are sub-leased to clients meeting the qualifications of the program.
Shelter + Care rental units are located in the City of Fort Dodge. Every effort will be made to match clients' housing type and location (e.g., proximity to: transportation, employment, grocery, medical, and supportive services, etc.).
Shelter + Care clients will be referred from various agencies throughout Webster County. Case management/supportive services will be provided by Webster County Disabilities Alliance.
Shelter + Care clients must agree to case management and a supportive services/case management plan. The case managers will develop individualized flexible service plans with the clients. They will also arrange for, coordinate, and monitor needed services. Clients must also sign forms needed to verify information in accordance with grant guidelines.
Shelter + Care clients must sign a lease for at least six months. The lease may be renewed with the mutual consent of the client, the case manager, and the Shelter Plus Care Housing Manager.
