Assistance provided to any adult who wants to complete Durable Power of Attorney for health care document. Once notorized or witnessed, document lists health care agent(s) who will make health care decisions when patient is not capable of doing so. The form also allows a person to state wishes for treatment in times of crisis and provide guidelines for agents and medical team. Presentations on advance directives also available to community groups, service clubs, church groups, support groups, etc.
