Program/Service Notes:
Provide training and support to individuals in their own apartments/homes, including money management, assistance with menu planning, shopping and housekeeping, monitoring medication, etc. Services are based on individual need. Individuals served generally have a diagnosed mental illness and/or have been referred for services by another agency, mental health professional or case manager. Funding is usually through the individual's county of legal settlement or Medicaid. MISSION STATEMENT: To facilitate partnerships with consumers to be successful and satisfied while learning, living, working, and socializing in the environments of their choice. Northwoods Living is a United Way agency partner.
