Program/Service Notes:
An assigned care coordinator helps the older adult and family assess the needs of the individual who is age 60 or over. They then develop a plan of care, arrange for appropriate services, advocate on behalf of the client and monitor services and changes in client needs to adjust the planned services as needed.
