REFERRAL or APPLICATION for Vocational Rehabilitation services may be made by contacting any of the area offices or service centers located throughout the state. Rehabilitation counselors may also be contacted during their regular visits to other state and local government offices or schools within local communities. ELIGIBILITY is dependent on the individual having a physical or mental impairment which constitutes or results in a substantial impediment to employment for that individual; a determination that the individual can benefit in terms of an employment outcome from VR services; and a determination that the individual requires services to prepare for, enter, engage in or retain gainful employment. Eligibility is determined by professional counselors who have a working knowledge of medical conditions, psychology, occupations, community organizations and resources. EMPLOYMENT is the objective for each individual served by the Vocational Rehabilitation Program. Eligible individuals receive counseling and guidance in choosing a vocational objective consistent with their interest and abilities and in determining how to prepare for and obtain a job. An employment plan is jointly developed by the individual and the rehabilitation counselor and summarizes the individual's job goal, services needed to achieve this objective, how services will be obtained and how and when progress will be measured. REHABILITATION SERVICES MAY INCLUDE: 1)Medical and psychological assessment 2)Technological assessment 3)Vocational assessment 4)Counseling and guidance 5)Physical and/or mental restoration services which may include therapy, wheelchairs, hearing aids, etc. 6)Special adaptive equipment or devices 7)Training for employment 8)Occupational tools, equipment or licenses 9)Planning for self-employment 10)Personal assistance 11)Supported employment 12)Job coaching 13)Job placement 14)Follow-up after placement to assist with worker and employer satisfaction Services may be provided after employment to maintain the job. INDEPENDENT LIVING services are also provided by the Division. Persons with severe disabilities who are not eligible for the regular Vocational Rehabilitation Program may qualify. Services provided must be designed to increase the individual's ability to function independently. Application or program information about independent living services may be obtained from any Vocational Rehabilitation Office. EMPLOYER SERVICES: Qualified job candidates. No cost consultation services for employment and disabilities, tax incentives, on-the-job-training, disability awareness training.
