In-home or community based activities that may include but are not limited to: individual advocacy community skills training services, personal environmental support, transportation & treatment services. Assist individuals with acquiring & maintenance of safe living arrangements, household budgeting, planning & preparation of nutritious meals and appropriate use of community resources. Development of communication skills; verbal & non-verbal. Skills assessment and care plan development; case management.
