The goal of Trinity Health Partners is to significantly improve the health status of our community through the promotion of health, wellness, self care and the prevention of disease and injury. Trinity Health Partners provides programs on health education, prevention and wellness and offers comprehensive health screening services to the general public.
Trinity Health Partners is a health prevention and wellness program provided by Trinity Regional Medical Center and supported by the Community Action Network. Trinity Health Partners offers a comprehensive, coordinated and collaborative approach for promoting and delivering health promotion, health education, wellness services and other health services to the community. Trinity Health Partners focuses on the citizens in our community who are not ill or injured with a mission of preventing illness and injury by providing outreach preventive services and health education programs, and promoting principles of self-care. Trinity Health Partners takes a leadership role in promoting wellness and healthy lifestyles and serves as a catalyst for new ideas and programs designed to help improve the health status of our community. It collaborates with other community organizations, agencies and businesses and encourages new partnerships to address community health issues.
